Per Neo-Penotran vag supp Metronidazole 500 mg, miconazole nitrate 100 mg. Per Neo-Penotran Forte vag supp Metronidazole 750 mg, miconazole nitrate 200 mg. Per Neo-Penotran Forte-L vag supp Metronidazole 750 mg, miconazole nitrate 200 mg, lidocaine 100 mg
Indications/Uses
Treatment of vag candidiasis (due to Candida albicans); bacterial (due to anaerobic bacteria) & trichomonal (due to Trichomonas vaginalis) vaginitis; mixed vag infections.
Dosage/Direction for Use
Neo-Penotran vag supp 1 supp at night & in the morning for 7 days. Neo-Penotran Forte & Neo-Penotran Forte-L vag supp 1 supp at night for 7 days. Recurrent or resistant cases Neo-Penotran vag supp 1 supp at night & in the morning for 14 days. Neo-Penotran Forte & Neo-Penotran Forte-L vag supp 1 supp at night for 14 days.
Contraindications
Hypersensitivity. Porphyria, epilepsy & serious liver function disorders. Virgins & young girls who are not sexually mature. Alcohol use during or at least 3 days after treatment. Disulfiram use during or w/in 2 wk of treatment. Pregnant women who have trichomonal vaginitis in the 1st trimester. Pregnancy (1st trimester) & lactation (breastfeeding can be started again 24-48 hr after end of treatment). Childn <12 yr.
Special Precautions
Discontinue use if peripheral neuropathy & convulsion (high doses & long-term use) occur. Do not use w/ contraceptive diaphragms & condoms. In patients w/ trichomoniasis, a concomitant treatment of sexual partner is necessary. Patients w/ serious impaired renal function requiring hemodialysis, hepatic encephalopathy. Do not use w/ other vag products (eg, tampon, douche or spermicide). Not recommended during menstruation period. May affect ability to drive or operate machinery. Pregnancy (2nd & 3rd trimesters). Neo-Penotran Forte-L Impaired renal/liver function.
Adverse Reactions
Vag irritation (burning, itching); headache, dizziness, vag discharge, vaginitis, vulvovag irritation, pelvic discomfort.
Drug Interactions
Metronidazole: Disulfiram-like reaction w/ alcohol. Increased oral anticoagulant effect, blood levels of phenytoin, lithium toxicity, plasma conc of astemizole & terfenadine; blood conc of carbamazepine. Decreased blood levels w/ phenobarb. Risk of CNS-related effects (eg, psychotic reactions) w/ disulfiram. Increased blood levels & neurologic effects w/ cimetidine. May increase blood levels & toxicity of fluorouracil; toxicity of cyclosporine. Increased cardiotoxicity w/ amiodarone. Interference w/ blood levels of liver enzymes, glucose (hexokinase method), theophylline & procainamide. Decreased levels w/ phenytoin. Miconazole: Increased risk of bleeding w/ acenocoumarol, anisindione, dicumarol, phenindione, phenprocoumon, warfarin; plasma conc & exposure to oxybutinin. May increase risk of phenytoin, phosphenytoin, cyclosporine, trimetrexate toxicity & cardiotoxicity w/ pimozide. May inhibit metabolism of astemizole, cisapride & terfenadine. Reduced carbamazepine metabolism. Increased or prolonged effects of opioid (fentanyl). May cause hypoglycemia w/ glimepiride. Reduced clearance & increased plasma conc of oxycodone. May increase bioavailability of tolterodine (in patients w/ deficient CYP2D6 activity). Lidocaine: Increased toxicity w/ antiarrhythmic products. Decreased plasma clearance w/ cimetidine & propranolol. Decreased plasma levels w/ phenytoin or barbiturates.
